Dr. Caine, the murdering dentist from the original picture, has escaped from the clinic where he was since being caught. Hoping to resume a normal lifestyle, he gets his approach to a quiet Midwestern town under a fictitious name and assumes the responsibilities of their town dentist Things are beginning to look up for Caine, until the afternoon after he grabs his new love from the arms of someone else. This pushes him back and right into the following rampage as from the first picture.
